Submit the following on the first day of class:
1. All Heath Standards Documentation
2. Copy of current CPR card
3. You will be given a General information packet that covers all required
information in detail.
HEALTH STANDARDS POLICY
1. You must submit evidence that you are in good health, have no
contagious diseases, and no physical restrictions with regards to this type
of job.
2. Medical documentation:
Hepatitis B Virus & Antibody blood test
Tuberculin (TB) skin test or chest film-current
Physical exam & medical history
These important documents are necessary to work in dialysis facilities.
Note: Students are also encouraged to be vaccinated with Hepatitis B
vaccine series prior to entering the dialysis program.
